After you sign up for a plan below, you will be contacted by Dr. Emma's support staff to pay your one-time annual enrollment fee and schedule your hour long initial appointment with Dr. Emma to complete any preventive care and exam needs!

Important Policy Member Info
Upon purchasing the plans above you will be billed separately for the enrollment fee of $100 and will be contacted to schedule your annual, 60-minute wellness exam. If you are a new patient to Dr. Emma, this will be an in-person or virtual appointment to establish care. If you are a return patient to Dr. Emma, this will be an in-person or virtual appointment to follow up on care.

All memberships are billed monthly. Memberships last for 12-months. You can cancel anytime, but will be billed for the reminder of the membership regardless of visit utilization.
You can upgrade your membership at anytime to a Silver, Gold, or Platinum plan by contacting our clinic and someone from our team will assist you with the switch. You are not charged a secondary enrollment fee for upgrading your plan. You can hold multiple memberships at once and many of our patients do to capitalize on savings and deals that these memberships provide.

It is your responsibility to keep track of service usage and scheduling of appointments. If you have ran out of appointments for the year and would like to or need to schedule another appointment you have two options. You can upgrade your plan by contacting us via above or you can schedule a standalone service and be billed for the fee-for-service rate listed on the website.

Keep in mind that if you upgrade to a membership plan, that means that you are waiving your right to have your insurance billed for medical appointments at EmPower U Health and Wellness LLC. You cannot submit these bills to insurance for reimbursement, but you may be able to use HSA funds depending on your membership.

There are some service items that are not included in Bronze, Silver, or Gold plans and would be an extra cost to you if used within a visit. There are memberships for some of these services that might cut down on costs as well if you utilize them frequently.

Labs and imaging can still be billed to insurance and costs for labs and imaging is not included in this membership. Lab draws are included in the membership and are no cost to you. If you do not have health insurance, Dr. Emma and her team will work with you to find low-cost alternatives in your area using our secondary lab and imaging suppliers.
